技術說明: 中醫使用針灸治病已長達兩千多年,WHO已認定且規範針灸可用於緩解64種疾病,其中使用針灸止痛是最常見的針灸適應症。然而針灸之所以能止痛常被質疑是一種安慰劑效果,針灸如何產生止痛療效仍是謎。雖然腦內啡學說曾被提出來證明針灸之止痛效果,但仍有無法以腦內啡解釋之針灸止痛效果。我們發現在老鼠進行電針灸,可以引起持續約一小時的止痛效果。而且這止痛效果是經由一個與腦內啡無關的全新作用機制。這個發現提供針灸具有止痛療效的有力科學證據,證實針灸的確可以生止痛效果,並不是一種安慰劑作用,因為老鼠不會預期接受針灸治療而讓痛覺降低。我們也發現此電針灸產生腦部的變化竟然與臨床上使用的神經刺激,有異曲同工之效。由於我們發現的這個針灸止痛效果與腦內啡無關,這在科學研究上是很大的突破。由於這個止痛效果不需腦內啡存在,將來也許可應用於對嗎啡已產生耐受性的癌末或手術後病人,提一種新的止痛療法。此研究也提供廠商轉譯契機,可開發攜帶式簡易刺激器,成為病人自我鎮痛利器。
 
Acupuncture has been used in Chinese medicine for over 2,000 years, and has been recognized by WHO as an useful regimen in relieving 64 symptoms. Among these, pain relief is the most frequent indication. However, the acupuncture-induced analgesic effect in humans is sometimes criticized as a placebo effect, and how acupuncture can relieve pain remains mysterious. Although the endorphin theory has been proposed, there are still many reports indicating that acupuncture can induce endorphin-independent analgesia. Using animal pain models, we found electroacupuncture can induce an analgesic effect lasting for about 1 hour via an opioid-independent novel mechanism. This finding provides scientific evidence supporting that acupuncture analgesia is not a placebo effect since animals would not expect to receive acupuncture therapy and express less painful response. Besides, we also found electroacupuncture and a clinical neuromodulation procedure share the same modulatory effect in the brain. The finding that acupuncture can induce analgesia via an opioid-independent mechanism is a scientific breakthrough. The nature of opioid-independency renders this electroacupuncture procedure a potential alternative strategy for pain relief in opioid-tolerant patients who suffer from postoperative pain or cancer pain at the terminal stage. Our finding also provides a translational potential in using a portable neuromodulator a patient-controlled pain killer.
